We are looking to recruit to an exciting new position to Adult Mental Health as a Mental Health Practitioner to be based at St Marys Community Health Campus in Portsmouth!

You will be joining our Community Mental Health Recovery Team which which is an established integrated multi-disciplinary team; we pride ourselves on being innovative, creative, and responsive to meet the needs of our service users.

The successful applicant will be a qualified Nurse or Social Worker and will be required to coordinate a caseload of service users of working age, providing comprehensive assessment and delivering evidence-based interventions to promote recovery from a range of acute functional illnesses. Excellent interpersonal and organisational skills are essential.

The role is varied with no two days the same and will include care coordination, assessments, risk formulations and management, medication management, provision of reports and mentorship. Risk assessment skills will be required and experience of working autonomously and within a team environment is also essential.
