Nurse Case Manager Band 6

Manchester Case Management (MCM) is an exciting new service being developed in Manchester, building on the successful High Impact Primary Care (HIPC) pilots and Active Case Management services.

MCM is a dedicated multi-disciplinary service pro-actively supporting people at high risk of hospital attendance and admission with a range of complex physical health, mental health and social needs. Working with the person and their family / carer, the MCM service focuses on what is important to each person in terms of improving their health and wellbeing. The multi-skilled team share the care of the person, and provide intensive and flexible support through clinic appointments, home visits and phone based consultations.

The MCM team includes GPs, Nurses, Social Workers, Pharmacists and Support Workers. GPs clinically lead the service which works with the wider Integrated Neighbourhood Team (INT) to draw in local community assets and services as part of the offer to MCM clients.

Working across the South Locality the Manchester Case Manager will undertake the care of a defined caseload of patients working closely with and based within one of our four integrated neighbourhood teams .You will have the relevant experience and skills of a case manager to undertake this role.

The role requires the post holder to hold a clean driving licence and have access to a car.
