Solent NHS Trust are looking for enthusiastic and capable individuals to join our Community Nursing teams as a Health Care Support Worker in the Portsmouth Community. The predominant base will be at St Mary's Community Campus in Portsmouth.

You will predominantly be working alongside Community Nursing staff to support patients who are in the last weeks/days of their life and who wish to die at home or in their place of residence. You will also work within the Community Nursing team to deliver general nursing care to housebound patients on the Community Nursing caseload.

The post holder will have excellent communication and interpersonal skills. You will need to be able to provide personal care along with other general nursing care such as catheter care and wound management. An important part of the role will be to offer emotional, physical and social support to patients and their families.Previous palliative care experience is desirable along with experience of working in a community setting although training will be provided.

The service operates between 08.30 -22.00, 7 days a week and therefore you will need to be flexible, adaptable and able to work the required shift pattern. This will include evening and weekend working with the flexibility to work long days when required.

Possession of an NVQ Level 3 in care is preferred and you will be required to drive and have access to a vehicle for work. Regular support, supervision and training will be given. An interest in working with palliative and older people in their own homes is essential.
