Following some exciting service redesign we are expanding our clinical team to include more Advanced Clinical Practitioners (ACPs). The posts are within the clinical hub with the current base being at Denton HQ from which staff are mobilised to areas of demand to provide scheduled and unscheduled care to cover the footprint of the organisation in and out of hours. You will be expected to carry out assessments via telephone and/or video consultation, provide patient care in treatment centres and also visit patients at home as part of the MVT (mobile visiting team)

The successful candidate will be qualified to MSc level in Advanced Clinical Practice or will be working towards credentialing/credentialed. Within the role it is required that you are a competent practitioner who can apply clinical reasoning to assess, diagnose, and prescribe where appropriate. In addition, you will discharge whilst making appropriate referrals and providing the right care and advice for patients with undifferentiated illness. You will support and contribute to the delivery of policy and procedures, provide leadership as required, will act as a mentor and positive role model to other staff and will facilitate training and education as required.

You will be supported by the clinical lead, the Head of Clinical Services and the Chief Nurse, in partnership with the Heads of Locality and Senior Operational Managers, to drive forward clinical excellence, leadership, quality, safety and high standards.
