Associate Nurse Practitioner/CNS

Salary up to £37,890 per annum, equivalent to Agenda for Change Band 6

37.5 per week to be worked flexibly in accordance with the needs of the service to include evenings, weekends and bank holidays.

About the Role

The post holder will be responsible for delivering palliative and end of life care as part of Mountbatten Hampshire Community Nursing services, providing holistic nursing assessment and interventions working towards advanced practice level, across all care settings. The role will promote a patients preferred place of care and advance care planning through the management of complex end of life care symptoms utilising your advanced skills and knowledge. The post holder will also need to manage staff, patient and carer expectations whilst meeting the integrated care needs of the patient. The role will require a collaborative approach to partnership working, both internally and externally, to develop and build relationships that are essential for excellent patient care.

About You

The post-holder will have First Level Registration with the Nursing and Midwifery Council and have the relevant degree in Health/Nursing studies. Experience in delivering palliative and end of life care is essential, together with holding a full driving licence. The post holder will demonstrable clinical skills and knowledge which will although them to provide the highest standard of care to their patients. They will be a strong team player who is enthusiastic and committed to their own personal development.
