Diabetes Care For You is a pioneering multi-professional community based service, consisting of Consultants, Nurse Consultant, Diabetes Specialist Nurses, Dietitians, Podiatrists and Psychotherapists.

We are seeking a dynamic and highly motivated individual with a proven track record of providing operational leadership. You will be a visible leader who is innovative, credible and resilient, with experience of operational management.

The team were finalists in the 2020 HSJ Value Awards and 2019 winners of a Quality in Care award, in the Mind and Body Health Together category.

The successful candidate will be responsible for providing operational leadership to support the delivery of high quality integrated care across Brighton & Hove and High Weald, Lewes & Havens to ensure we achieve our trust objective of ‘excellent care at the heart of the community’.
