We're looking for a Community Staff Nurse to join our Community Primary Care Network (PCN) Nursing team in Crawley its surrounding areas, including Copthorne and Horley.

Our team works closely with our linked GP partners, social services, specialist nurses and local hospices to deliver excellent care in the community. We are well supported by a Trust which understands community nursing and delivers adult nursing care to patients across Sussex.

Your responsibilities will be:

  • Visiting patients in their own homes and providing excellent evidence based nursing care. This will usually mean working alone, although you will have support from a well-established and experienced team.
  • Carrying out nursing interventions such as wound care, medicine administration, diabetic management, end of life care and syringe drivers, along with any other nursing interventions provided by the community team.
  • Documenting all interventions and relevant information on our electronic patient record system (SystmOne).
  • Supporting the caseload managers in ensuring the smooth running of the office and the team.

You must be able to travel around the Trust in a timely manner, so must have a full driving licence and the use of a car for work purposes.
