An exciting opportunity has arisen to recruit PCN Clinical Leads to enhance the leadership structure across health, social care and the voluntary sector in the West London footprint.

 

About Us:

Role:

The PCN Clinical Leads will be responsible for the clinical, strategic and operational management of the community Integrated Care Team (ICT) across one of the five Primary Care Networks (PCN) in West London Clinical Commissioning Group (CCG) with the primary aim of improving integration of services across health, social care and the voluntary sector.

 

In addition to managing the operational teams, the post holder will work closely with the Clinical Director of the PCN and coordinate delivery and development of the integrated care service in the community. The post holder willhave direct accountability to CLCH as well as being responsible to the PCN Clinical Director. The post holder will provide support to the PCN CD in delivering local priorities and developing local pathways.

 

The PCN Clinical Leads will work collaboratively with system partners in the development of these integrated care pathways wrapped around the PCN to meet local population health needs, ensuring that services are equitable and effective and local and national standards of practice are maintained. The successful candidate must be comfortable working with different organisations with competing priorities and be able to work collaboratively to creatively solve problems and agree positive outcomes. They must have a proven track record delivering service improvement using QI methodology. The post holder should be able to demonstrate good knowledge and understanding of population health and work with the integrated team to identify priorities and areas of improvement for their dedicated PCN
