Leicestershire Partnership NHS Trust (LPT) provides a range of community health, mental health and learning disability services for people of all ages. Delivered through over 100 settings from inpatient wards to out in the community, our 6,500 staff serves over 1m people living in Leicester, Leicestershire and Rutland.


Due to some exciting changes within the trust and in response to the Patient Safety Strategy, Leicestershire Partnership NHS Trust (LPT) have created a number of Patient Safety Incident Investigator roles to work within directorates. These are brand new roles for LPT and we are looking for investigators to complete incidents investigations and complex complaints in addition to supporting the trusts initiative to improve the involvement with our patients and their families through our review process.

We would welcome applications from staff with Clinical qualifications (i.e. Registered Nurse, Midwife, AHP) or Police, Armed Forces, Aviation, Fire and Rail Accident Investigator, Healthcare Patient Safety, Complex Complaints ,Health & Safety Investigator related roles that can demonstrate equivalent experience and transferable skills.

We are looking for an applicant that will support improvements in the quality of clinical care, systems and patient safety through development of safety recommendations as a result of investigations and through sharing, evaluating and monitoring the effectiveness of lessons learned.

The successful applicant will undertake robust patient safety incident investigations (PSII’s) by working with a range of internal and external stakeholders to investigate, analyse and use system thinking and human factors approach through the collation of evidence and findings using the agreed methods within the Trust’s Patient Safety Incident Response Framework (PSIRF) and where required complex complaints .

For areas where our teams work to staffing rosters, rosters are set in advance at a team level and you will be joining an established roster.
