Hours of work:  22.5 per week

Due to the success and development of community services within North Lincolnshire the Single Point of Access Service which incorporates the Virtual Ward has now expanded to include 24 hour cover. We are looking to appoint staff nurses into our friendly multi-disciplinary team who work closely with other community services to provide wrap around care for patients registered with a North Lincolnshire GP. The role could be full or part time based on preference and includes a mixture of shift patterns that will be rotational to cover a 24 hour, 7 day a week service need. We would consider a request from interested individuals to work out of hours only which would include evenings from 1800hrs, weekends, bank holidays and nights.

The post will suit staff nurses who enjoy working within a developing service and are motivated and enthusiastic. This post is telephone triage and the post holder is not expected to deliver face to face care. Therefore, this role will provide working arrangements which may suit an individual who’s personal circumstances or career needs do not facilitate them providing face to face care.We are keen to receive applications from people who come from a Black, Asian or other minority ethnic background or identify as having a disability

The purpose of the Single Point of Access Service (SPA) is to signpost patients who require health and social care services to the most appropriate service that meets their needs.

The SPA service aims to:

  • Offer an alternative to secondary care where safe and appropriate
  • Assist in the reduction of the length of a hospital stay by ensuring patients are signposted to the most appropriate place
  • Assist in the management of patients accessing the GP Out of Hours Service which includes receiving and managing referrals from NHS 111
  • Provide a telephone triage service and/or a secondary assessment for patients who have ongoing health needs and signpost to the most appropriate service or provide self-care advice

The virtual ward describes an enhanced package of healthcare provided with the aim to maintain care within the home wherever possible. It is an alternative pathway for those patients:

  • who do not require acute medical in-patient support and are clinically fit for discharge, but do require on-going nursing interventions and care and/or intensive therapy
  • where admission to an acute hospital can be avoided through the provision of intensive therapy and/or nursing interventions and care

NB Please note the job description attached does not include rotational working and there is an expectation that the staff employed will be employed on a rotational contract.
