Continuing Nurse Coordinator (Learning Disabilities)

An exciting opportunity has arisen for an experienced LD nurse to join the Nottingham Citycare Partnership Continuing Care Team as a Continuing Care Nurse Co-ordinator. Nottingham CityCare Partnership is the leading provider of NHS community services in Nottingham; a staff-led social enterprise; trusted to connect with and care for people, to improve the health and wellbeing of the community.

Nottingham CityCare is looking to recruit a forward-thinking, resourceful, experienced and confident nurse to the post of LD Continuing Care Nurse Co-ordinator. The role of the nurse Coordinator will be to facilitate the implementation of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care within Nottingham City.

The role will involve providing advice, undertaking health needs assessments and coordinating continuing healthcare funding. You will carry out work autonomously and in partnership with health and social care professionals in arranging care packages and placements. You will be part of the Continuing Care Team which is based at Aspect House. The Co-ordinator will work in the Nottinghamshire community and also be required to cover the Nottingham hospitals.
