An exciting opportunity has arisen within Burgess Hill and Villages Primary Care Network (PCN). We are looking for Community Staff Nurses to join our team covering Burgess Hill, Hurstpierpoint and its surrounding villages.

Our team works closely with our linked GP partners, social services, specialist nurses and our local hospice to deliver excellent care in the community. We are well supported by a Trust that understands community nursing, and delivers adult nursing care to patients across Sussex.

Responsibilities include:

  • Providing nursing care for patients in their own homes, including tasks such as wound care, medicine administration, diabetic management, end of life care and syringe drivers, along with any other nursing interventions provided by the community team
  • Assessing, planning, implementing and evaluating patient-centred care plans
  • Documenting all interventions and relevant information on our electronic patient record system, SystmOne
  • Supporting the caseload managers in ensuring the smooth running of the office and the team
  • Supporting and coaching students and other junior staff
  • Triaging patients

You must be able to travel around the Trust in a timely manner, so you will need to have a full driving licence and the use of a car for work purposes.
