Rochdale HMR Integrated Neighbourhood Teams

Rochdale HMR is a dynamic and forward thinking Borough and we are excited to be able to offer the appointment a part time band 5 Community Staff Nurse.

You will be based within the out of hours team and in partnership with GP’s and the wider Primary Care Team, manage the health needs of both house bound and non-house bound patients across the community of Heywood, Middleton & Rochdale. You will work under the direction and supervision of the team leader to provide care to patients with complex and non-complex needs with the aim of promoting independence and self-management, whilst preventing unnecessary admissions to secondary care.

Excellent communication skills and the ability to work independently within a variety of community environments will be crucial to the success of this role.

In addition you will have opportunity to contribute to the ongoing transformation and on-going development of our Integrated Neighbourhood Teams.
