East Cheshire NHS Trust are looking to recruit a Lead Advanced Care Practitioner for the CHAW Care Community supporting the communities of Chelford, Handforth, Alderley Edge and Wilmslow.

The CHAW Care Community team are an integrated multidisciplinary team of professionals namely Nursing, Physiotherapists and Occupational Therapists. The team have close links and work collaboratively with the wider care community services such as the local authority and voluntary sector. The Care Community is aligned to the CHAW Primary Care Network.

The Lead ACP will provide clinical leadership for the integrated team in particular the 'Home First' visiting service. Your clinical skills will contribute to enable people presenting with health/social crisis to remain safely and supported in their own homes and prevent avoidable Accident and Emergency attendances. The service  also supports the Discharge 2 Assess initiative to provide timely assessment for rapid hospital discharge.

The Lead ACP will work alongside the Care Community Coach and Clinical Lead to work strategically, contributing to the planning and development of integrated services, acting as a driver for change and ensuring standards of care and best practice in line with up to date evidence base.

You must be able to travel around community settings and demonstrate this in your application.

An MSc Advanced Clinical Practice is required for this role however, applications will also be considered for those within the last year of a relevant course to join on a Developmental pathway offering initially at a Band 7 progressing to 8a following completion of all of the required skill set outlined within the Job Description and Person Specification.

Hours per week: 37.5 over 5 days (part time hours will be considered).
