Norfolk Community Health & Care NHS Trust have an exciting opportunity available for an experienced Band 6 Occupational Therapist / Physiotherapist who enjoys working in partnership with patients and their families to join our successful Community Response Team working alongside our Intermediate Care Service supporting patients in their own homes.

This large team consists of Occupational Therapist, Registered Nurses, Assistant Practitioners and Healthcare Assistants. The team work from St James Clinic, Kings Lynn covering the West of Norfolk. You are required to have community experience and able to triage, and review referrals received from the Acute setting under the D2a pathway, Paramedics and General Practitioners also Community Matrons. Also assess and support staff to ensure early discharge from the acute setting or admission avoidance in non urgent cases, also support the precured beds in nursing homes.

The ability to effectively co-ordinate and communicate often complex and diverse information is vital in the effectiveness of the service. You will be required to triage, review and initiate timely responses to received referrals to ensure patients in their own homes are provided with an excellent and safe service.

If you are motivated and passionate about providing excellent patient care then this is the post for you. As a Clinician working in the Community Response Team and Intermediate Care Service the role will require an ability to work closely with professionals from other organisations to enable integrated working. As part of the team you will work collaboratively to complete assessments, interventions and demonstrate sound clinical reasoning to enable patients to achieve optimum potential during their stay on our case load.

Clinical supervision will be provided, we offer the opportunity to work in an experienced and supportive team with opportunities for ongoing professional development.

Successful Candidate should be/have:

  • A registered Occupational Therapist / Physiotherapist with the HCPC (Health Care Professions Council)
  • Working as Band 5 or Band 6 OT
  • An understanding of Occupational Therapy process / interventions and can take a ''hands on'' approach
  • Excellent communication and time management skills
