Department: Adult Services Localities - Southampton
Hours: Minimum of 22.5 per week. Full and Part Time Available
We are seeking experienced nurses to provide complex care and treatment to both adults and older people living at home who have long-term or complex health needs. You will work alongside a number of specialist community services including Palliative Care, Proactive Nursing Services, Bladder and Bowel, Respiratory, Diabetes as well maintaining established pivotal relationships with our primary care colleagues.
You will work as part of a motivated and caring team who deliver exceptional high-quality healthcare, which keeps our patients at the heart of everything we do.

About You
The ideal candidate will be dedicated, highly motivated, and committed to providing high standards of care to people within their own homes and the local community. You will have excellent communication skills as you will be undertaking complex nursing assessments, you will also be required to build relationships within the specialist teams to help deliver a high-quality service. Experience in community nursing is desirable for this post but not essential as full training will be provided, and the post holder will be supported to develop in the role. You must have a driving license and access to a car for work purposes, you will have the opportunity to apply for a lease car.
