Are you a BANK Band 6 Community matron?
We are currently looking to recruit an experienced Bank community matrons to support care homes, within Kensington & Chelsea, hours will be Mon-Fri.
So what will you need to join our BANK as a Community matron ?
A registered nurse with advanced clinical skills, Evidence of recent post-registration nursing experience within the Community setting, Ability to communicate effectively both verbally and in writing, significant knowledge and experience in the clinical care of adults and older people in a range of healthcare settings and the management of long term conditions.
As a Community Matron you will be self-motivated with strong organizational skills and able to make complex clinical decisions autonomously as required. You will have Experience of undertaking comprehensive health needs assessments and writing care plans and manage caseload patients.
As a Community Matron you will use your advanced clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce likelihood of unnecessary admissions to hospital.
This is an exciting post, with the opportunity to help make a real difference to patients and a chance to develop both clinical and strategic skills.
The post holder will be expected to work Monday-Friday across care homes in Kensington and Chelsea and North West Westminster
