Integrated Care System Advanced Clinical Practitioner Lead

 

Location: Fanum House, 484 Derby Road, Nottingham, NG7 2GW.

Salary: £63,751 pro rata per annum

Hours: Part time – 22.5 hours per week (Monday to Friday)

Contract: Contract of Employment – fixed term for 12 months/ secondment opportunity.

On behalf of Nottingham and Nottinghamshire Integrated Care System Nottinghamshire Alliance Training Hub (NATH) is looking to appoint an Integrated Care System Advanced Clinical Practitioner Lead.

The post holder will be responsible for leading on the implementation of the Nottinghamshire Integrated Care System (ICS) Advanced Clinical Practitioner Agenda. They will support the ICS ACP steering group in developing and implementing a multi-professional ICS advanced practice strategy. The post holder will be supported by the ICS primary care ACP lead and the Frailty ACP scoping post.
