Are you looking for a new challenge in a constantly evolving role? An exciting opportunity has arisen for a Triage Coordinator in SPUR / Gateway.
SPUR / Gateway is a clinical and customer service team employed to screen, triage and process referrals for people who require health or social care services. We are office based in Leeds City centre.
The role is fast paced and will involve clinical triage of referrals predominantly urgent in nature from both primary and secondary care. Referrals come from a variety of sources including hospitals, GPs, YAS, social workers and other health and social care professionals
If you are able to adapt to change and have relevant clinical triage / decision making skills then this could be the post for you.
The successful candidate will need to possess an in depth knowledge of complex clinical health conditions and social care issues and be able to use clinical judgement skills to identify what intervention is required based on the health need / presenting problem.
The role is very demanding therefore the ability to be calm under pressure in addition to being able to prioritise work is essential. The post holder will also need to possess excellent communication skills as the role involves communicating with staff and professionals at all levels.
You will also need to be able to use the in house telephony system and have IT skills as we have access to and use systems including System1, Leeds Care Record and CIS in addition to our own electronic files.
If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
