A fantastic opportunity has arisen to join Thames Medical community nursing team.

We are looking for a District nurse with the specialist practitionner qualification.

You will be joining an extremely friendly, busyand committed team. You will be responsible for holding a caseload and supporting junior members of the team. You will be required to have excellent communication skills to enable you to liaise with your assigned GPs and other members of the multi-disciplinary team. You will be required to assess, plan and evaluatecare of the patients within your caseload.It is expected that you will conduct regular 1:1's and appraisals for other members of the team. You will also support and develop those team members and student nurses when required. Audits will be completed as required and you willreview incidentsthat have occured.

You will have the support of experienced team leaders who will encourage your personal and professional development .
