Hospital Palliative and End of LifeCare Discharge Coordinator

AfC Band 6,Salary up to £37,890.00 per annum

37.5 hours per week(may include weekend and bank holidays)


About the role:

An exciting opportunity has arisen for an experienced, enthusiastic registered nurse who is seeking to make the next step in their palliative career to join our team based at University Hospital Southampton Trust.

As the new Mountbatten Palliative Care Discharge Coordinator, you will actively support discharge planning and facilitation, you will help to prevent inappropriate admissions into hospital and ensure that care at home is a viable option for people approaching the end of life.

Working in partnership with the hospice and hospital teams, consultant nurse (palliative care), social care, primary care, nursing & residential homes, and NHS continuing healthcare in facilitating integrated patient focused discharge provision. Whilst an employee of Mountbatten, you will be a core member of the Hospital Palliative Care Team based within University Hospital Southampton.

About you:

You will have experience of collaborating with other health and social care providers in primary, secondary, independent, and voluntary care is essential. You will ensure communication links are in place to deliver a continuous, well-coordinated and seamless discharge process as well as tracking those patients appropriately admitted into the hospital.

An effective advocate for the patient, you will understand the needs of those receiving palliative/end of life care as well as governance issues, risk management and audit procedures.

You will hold current registratoin with the NMC, and hold relevant nursing degree.
