We are offering an inspiring opportunity to join our innovative team as an Advanced Clinical Practitioner for Frailty. Potential post-holders are invited from nurse or AHP backgrounds to work with patients in their own homes and in community hospital settings. You will work for Solent NHS Trust Adult Services Portsmouth Service Line if successful.

You will be part of a highly skilled, and genuinely supportive team of practitioners for frailty. The focus of your role will be predominantly clinical. You will employ advanced level clinical practice using physical assessment and diagnostic decision making to develop a management plan that you will enact with multidisciplinary colleagues.

Our aim is to ensure patients with frailty are supported to optimise their health and independence through a combination of expert clinical care and facilitated self-management.

You will have the exciting opportunity to contribute to the development and management of a new care model that will uplift our current provision and reduce unnecessary hospital admission, as well as facilitate discharge.

This is a senior post and you will be required to demonstrate or develop capability across the four pillars of the advanced practice framework. To achieve this, you will be offered support by a Consultant Practitioner and senior managers. There is a competency framework in development, and you will receive specialist training to develop your skills. This will include internal and external training opportunities as required.

Solent NHS Trust manages community services within the City of Portsmouth, playing a pivotal role in integrated Health and Social Care delivery in intermediate care settings working alongside Primary Care and Acute system partners. Solent NHS Trust is rated as ‘good’ by CQC with a strong drive for excellence in patient care balanced with a real focus on the wellbeing of individual staff.

You will already have experience at senior level in working with people with frailty. Applications are welcome from people who already have the competency to work at advanced practitioner level with this group of patients. They are also welcomed from people with the dedication and passion to develop the skills to work at this level but who may not yet have all the capabilities described in the person specification.
