SENIOR COMMUNITY NURSE – BAND 6 - 30 hours per week

Would you like the opportunity of working in a friendly and supportive Community Nursing Team in West Norfolk?

An exciting opportunity has arisen for a motivated, experienced nurse to come and join us within the Coastal PCN Team based at Heacham Nurses Base, Heacham.

The Senior Community Nurse as a Case Manager has overall responsibility for a designated caseload and manages care delivery by the integrated team. The post holder will also be responsible for staff management including appraisal, supervision and staff development.

You will be an experienced community nurse with a wide range of clinical competencies and a mentorship qualification. Clinical support will be provided by the Clinical Leads and you will be supported to develop your case management skills.

You will have good communication skills and be able to deliver high quality evidence based nursing care within the community setting to meet the needs of patient who are housebound.

You will also be able to work autonomously and be able to work in an integrated way with our partners in health and social care.

The service operates between the hours of 08:00 – 20:00 hours, and you would be required to work shifts between these times.
