We are looking for a dynamic Occupational Therapist to join the Home Assessment Reablement Team (HART) to deliver the ‘Discharge to Assess’ programme. This would involve you assessing patients who have been recently discharged home from bed based care in Oxfordshire, writing goals and care plans and working with the Reablement Support Workers, Assessors and other agencies to improve patients’ independence, reduce their care packages and help prevent hospital admissions.

You will need to be able to lead and motivate both patients and staff to embrace the reablement model of care and may be involved in training staff in these principles.

You will need to have some community based experience and have a wide range of skills to carry out high quality home assessments and interventions. Having a good knowledge and experience of working with equipment to help aid independence and care is also essential.

We are looking to recruit to our South Oxfordshire team (base Abingdon Health and Wellbeing Centre).

The service operates 7 days a week between the hours of 7am-10pm.

It is essential that you have a full UK driving licence and your own vehicle, due to this being a community based role. Mileage is paid at 45p per business mile.

As part of Oxford University Hospitals NHS Foundation Trust, you will have regular supervisions, as well as training and professional development opportunities and interaction with other teams and therapists working in the hospitals.
