Social Prescribing Link Workers

Salary: Up to £24,000 WTE per annum

Location: Parkwood PCN ,Northamptonshire

This is an exciting time for General Practice. All practices within the county have come together to work in Primary Care Networks (PCNs). These are groups of GP practices working in partnership to support their local population.

The aim of this new role is to empower people and help them take control of their health and wellbeing through referrals to non-medical interventions and signposting other available support services. You will spend time with people to help them focus on ‘what matters to me’. You will take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

Social prescribing can help to strengthen community resilience and personal resilience, and reduces health inequalities and support people with long-term conditions and those who are lonely or isolated, and ultimately have a positive effect on wellbeing.

To apply for this post you require:-

To be Qualified to NVQ Level 3, Advanced level or equivalent qualifications or working towards.

Ideally, have experience of:-

working directly in a community development role possibly, adult health and social care, learning support or public health/ health improvement (including unpaid work and/or experience of working with the VCSE sector

data collection and analysing information to assess the impact of services

using a range of IT health data base systems, including advanced skills in word processing, Microsoft packages including outlook and using the internet to create plans, reports and presentations.

The ability to:-

  • listen, empathise with people and provide person-centred support in a non-judgemental way
  • communicate effectively, both verbally and in writing, with people their families, carers community groups, partner agencies and stakeholders
  • access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes
