Sussex Community NHS Foundation Trust (SCFT) is the largest community healthcare provider in Sussex.
An exciting opportunity has arisen to join our multidisciplinary community team as a Community Sister. This includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The role is that of a caseload holder, providing clinical case management for patients with complex health needs who are at risk of deterioration which may result in declining quality of life, avoidable hospital admissions, or an unnecessary length of hospital stay.
The role includes using your clinical skills for assessment, diagnosis, treatment and/or appropriate referral in collaboration with other agencies to ensure care coordination, and also includes being the named key link for individual GP practices. As an experienced clinician you will provide leadership and mentorship to other more junior members of the team, and have delegated responsibility for a team of nursing professionals.
The role is based in an integrated hub of experienced and supportive nurses, and has direct access to the specialist teams within the hub.
