The Regis Community Primary Care Network (PCN) team are looking for an experienced community nurse who can lead and support a clinical team delivering care to patients in their own homes.
You will need:
The Clinical Nurse Lead is an exciting and autonomous role supported by the Operational Team Lead and the Practice Development Nurses. It plays a key role in supporting staff to develop and maintain their clinical competency, ensuring teams deliver the right care at the right time to the individuals on the community nursing caseload.
The successful candidate will hold a Community Specialist Practitioner qualification or its equivalent. They should be able to demonstrate excellent communication and negotiation skills, as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the PCN. The Clinical Nurse Lead will be required to support the community nurses and the PCN partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.
The Regis Community PCN team are based at Bognor Regis War Memorial Hospital. There is free parking on site.
