For this post we are offering all registered nurses an attractive employment package, including a welcome payment of £2,500 (paid in two instalments during year one for new LPFT employees) and £4,000 payment in lieu of relocation (to assist with commuting) or full relocation package** In addition there is a whole host of unique staff benefits and educational support as part of a nurse’s CPD.

Maple Lodge is a 15 bedded Male / Female open rehabilitation unit who provides care to adults from 18 with no upper age limit with a range of mental health needs. Maple Lodge is AIMS accredited with the Royal college of psychiatrists. We provide care to both informal and patients detained under the mental health act.

Our ethos is to offer a timely assessment to inform an assessment of needs in psychological intervention, functional skills and social needs. Care plans are navigated around these core needs and discharge planning and accessing the local community is key to the care we provide at Maple lodge. This allows us with our shared vision to support our service users to live well within their communities
It is an exciting time to work within rehabilitation services within LPFT, currently our service is undergoing a program of transformation, working closely with our colleagues within community rehabilitation services, re-enablement, locked rehabilitation services, forensic services and our acute care colleagues to offer the least restrictive interventions and evidence value added to a person care for each day they remain in hospital
The successful applicant will act as a care navigators to named inpatients to formulate plans of care and discharge planning, coordinate the care program approach whilst working with other key professionals and services. Our aim is to enable our service users to reach their optimum functioning to enable them to live well within their community, engaging with key professionals, working part of a variety of multi-disciplinary members of the team as well as supporting service users and their carers.
The successful applicant in addition to care navigation will be responsible for shift coordination, delegating and allocating tasks throughout the working day to other team members.
Good organisational skills and IT skills are essential.
You will need to demonstrate a commitment to providing a high quality standard of care, excellent communication and interpersonal skills and a positive attitude towards change and an ability to contribute to the evolving nature of the service. You will receive regular supervision both management and clinical. There is an expectation that as a qualified member of staff that both management and clinical supervision will be delivered to less senior staff. There are training opportunities available to support growth in the role. You will be expected to work shifts including weekends, nights and bank holidays over a 7 day period.

We offer 24 hour services within our Inpatient, Crisis and SPA teams, if you are successfully appointed to posts in these areas or others that might have specified, you will be required to work unsociable hours and shift patterns.

Preference will be given to NHS colleagues who are currently 'at risk' and meet the minimum criteria of the Job Description & Person Specification.
