Band 2 – Health Care/Rehabilitation Support Workers. 20 hours (annualised hour’s contract).

An exciting opportunity has arisen for Health Care/Rehabilitation Support Workers to be part of the newly developed Integrated Neighbourhood teams in Barnsley.

Successful candidates will gain valuable experience working across the Intermediate Care pathway within the Neighbourhoods. Successful candidates will be working across the Barnsley area therefore being a car user and driver is essential to the role.

Support workers will work as part of a multi-disciplinary team of nursing and allied health professionals who work across the Intermediate Care pathway in community. Our Neighbourhood and Rehabilitation and Crisis Response service provide a 24/7 service which includes:

  • Neighbourhood nursing and support offered as an alternative to hospital admission.
  • Out of hour’s Neighbourhood provision
  • Early hospital discharge support for patients requiring intravenous antibiotics
  • Multi-disciplinary assessment and rehabilitation.

The teams provide assessment, treatment and support within patient’s own homes, Independent Sector Care Homes and spot-purchased nursing care homes, based on patient need and dependency levels.

It is essential that candidates have the ability to use their own initiative, have good communication skills and are able to manage their time effectively.

The use of a vehicle to undertake any necessary travel in connection with the duties of the post is essential along with the ability to work flexibly including shift work, weekends and bank holidays.
