We have an exciting opportunity for motivated lifestyle practitioners who are enthusiastic about diabetes to join our supportive and dynamic clinical team.

We have an opportunity for
. a Band 4 Lifestyle Practitioner to work on the LEEDS Programme delivering group based education

What will you do?
The LEEDS Programme is a structured education 3 week course delivered to patients with Type 2 diabetes and forms part of the service delivered by the Community Diabetes Team. The programme is currently being delivered virtually due to the pressures of COVID-19. We are looking for someone who is able to adapt to both virtual and face to face group delivery.

The community diabetes multi-disciplinary team consists of specialist nurses, dietitians, GP's, mental health practitioner, lifestyle practitioners and a clinical support worker who provide care and treatment to patients with Type 2 diabetes.

Your main role will be delivering the group structured education programmes across Leeds in various venues and virtually. We are looking for an excellent lifestyle practitioner to work alongside other members of the team. It's our priority to continue delivering the high quality diabetes care for which our service is known. You will have the opportunity to work alongside experienced staff that can help you develop your knowledge, skills and experience. The role does involve occasional evening and weekend work in order to meet service demands.

About You
You must have NVQ level 3 or equivalent and CCSE English. Health Coaching training, experience of working in groups and within a dietetic team is desirable and a commitment to high quality patient care along with excellent clinical skills and relevant experience. Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary adjustments can be considered in accordance with the Equality Act 2010.
