£9.12 - £10.25 pr hr (FTE £17,783 - £21,058)plus 12% holiday payalong withenhancements for night, weekend and bank holiday working.

Are you an experienced adult Health Care Assistant looking for a new challenge? We are recruiting for aqualified Adult Palliative Care Support Worker (PCSW), who will be dedicated to providing professional and compassionate healthcare support to patients at home, who are in the last months or days of their lives.

Together withsupervision from the multi-professional palliative care team or primary care team, the PCSW will participate inthe immediate provision of individualised nursing care, which aims to avoid unnecessary hospital/hospice admission.

The successful candidatemust have the ability to work alone and as part of a team; hold a current UK driving license, have access to their own car,andwill be required to undertake an enhanced disclosure and barring services check.(DBS)
