An exciting opportunity has arisen for a highly motivated and experienced Advanced Clinical Practitioner to join our expanding and innovative community Urgent Care Service.

We are seeking dedicated Advanced Nurse Practitioner with Advanced Physical Assessment and Non-Medical Prescribing qualifications who is committed about working within the field of community urgent care. This is a full time post covering Worthing and Chichester.

The post holder will work as part of the team in the assessment, diagnosis and treatment of patients who are referred to Responsive Services with a range of care health needs. They will autonomously manage the episode of care within their scope of practice and experience, liaising with primary care, acute and community colleagues as appropriate, and utilising the skills and competencies of an advanced nurse, such as prescribing.

In return they will be supported by the Urgent Care Service, receive clinical support from an appropriate clinical professional, managerial supervision from the team manager, and an opportunity to expand upon existing skills.

IMPORTANT: The role will involve regular travel across multiple sites in West Sussex, and the post holder must be able to meet the travel requirements of this post. Please indicate on your application how you can meet this requirement.
