With a budget of around £270m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

We are inviting applications for a highly motivated and dynamic Macmillan Health Care Support Worker to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients own homes and residential care homes. The post holder will have a high level of knowledge in the field of community care and will have a specific interest in areas such as palliative care, long term conditions, catheterisation and wound care. You will be required to work autonomously to provide a whole person approach to the care and management for patients within this setting.

The post involves liaising with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to this group of patients and their carers.

We require a Health Care Support Worker with NVQ level 3 in health and social care who has excellent communication skills and is confident working across professional boundaries. You will need to be flexible, adaptable & a good team player with the ability to work independently within both the Hartlepool and Stockton community settings. Please note that the working patterns are across a 7 day period and may require the post holder to work across both a day and night shift pattern.
