We have an opportunity for a Social Worker (Social Care Assessors) on a full time permanent basis within the Ryedale North Planned Care Team.

Our skill mix is a 2:1 ratio of qualified practitioners, in recognition of the increasing complexity of clients in Adult Social Care. We fully recognise the need to support staff development and enhance skills and experience which we deliver through senior professionals within the service.
We have proud to have made great staffing additions to our teams, which provide us with the experience, skills and creativity of both long established colleagues and newcomers to the county. As a Social Worker you will undertake a range of asset and strength based assessments and interventions to ensure that adults with care and support needs and their carers achieve their desired outcomes and wellbeing to live independent lives. This will also include developing personalised care and support plans that represent best value. You will determine eligibility for adults with care and support needs against the national standards and ensure they are safe from harm. Where harm or abuse is identified you will ensure the safety of the person and undertake the relevant safeguarding enquiries.

You must have a good working knowledge of current adult social care legislation and application of the legislative principles including the Care Act 2014 & Mental Capacity Act 2005.
