The Frimley Integrated Care Systems (ICS) Strategic Ambitions developed by all the health and social care partners across the footprint has set out how we will redesign services to deliver the Long Term Plan in Primary Care. As an Integrated Care System our plan sets out how local services will evolve and become sustainable over the coming years – ultimately delivering better health, better care and improved efficiency.

The strategic vision is to continue to develop an advanced digital ecosystem that is driven by resident needs, clinical, operational and transformation priorities to support information being available to healthcare professionals at the point of care and residents when needed.

Five roles are being recruited for each of the places within Frimley CCG collaborative. These roles will offer an exciting opportunity to work with in the Frimley Health & Care ICS, with its partner organisations and work in primary care leading on transformation change. An ability to liaise with our local clinicians and where appropriate, patients and public, will be key to the success of these posts. The post holder will work alongside the ICS Chief Information Officer, Associate Director for System Transformation , Digital Primary Care Programme lead and Frimley CCG Collaborative leadership. Frimley CCG Collaborative is formed from East Berkshire CCG , North East Hampshire & Farnham CCG and Surrey Heath CCG. These are divided into five pla ces (Bracknell Forest , North East Hampshire & Farnham , Royal Borough of Windsor & Maidenhead, Slough, Surrey Heath). Each of the roles advertised will be assigned to one of these places.

Furthermore, the post holder will work with colleagues to provide assurance, through accurate reporting, to the ICS Digital Programme Board regarding the delivery of key digital projects assigned to them.They will have the responsibility to ensure that Digital First Primary Care projects are run to a high standard, have clear objectives and are aligned to the ambitions of the ICS and Frimley CCG collaborative.

The successful candidate is expected to have transformational change management experience, good leadership, management skills, have a knowledge and experience of working primary care and STP/ICS’s.
