Complex Care at Home Team

Band 3, Health Care Support Worker

Base: Blaenau Gwent & Monmouthshire

Are you someone who has an interest working with individuals with complex health needs who wants an exciting new challenge?

Enthusiastic and suitably experienced Health Care Support Workers are required to provide care for individuals, living in their own homes, who require evasive mechanical ventilation within the Blaenau Gwent and Monmouthshire Area. You will be working alongside experienced Assistant Practitioners and your role would be providing support and assistance with activities of daily living for these individuals.

HCSWS should have knowledge and experience of working with patients with complex health needs. We welcome HCSW’s to apply who would be willing to learn and undertake a comprehensive induction. This role would include rotation shifts to meet the needs of the service and ensure skill competency is compliant.

Successful staff will be expected to enable the individuals to live a full and productive life supporting physical and psychosocial needs, this includes using individual’s vehicles in the community to assist them in attending appointments and social activities. This unique opportunity allows the nurse to develop a relationship with the patients to know their individual needs and health needs to provide high, quality nursing care.

This is an exciting time to become a part of the Complex Care at Home Team. The service is an integral part of clinical futures and places high importance on dignified care at home. The service is driven towards succession planning and there are a variety of training and development opportunities available for HCSW’s within this unique service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
