We are looking for motivated and flexible individuals with experience in rehabilitation to join the Southampton Community Independence Service as Associate Practitioners/Senior Rehabilitation Assistants.

The Community Independence Service has 3 multidisciplinary teams incorporating health and social care working in different localities across the city. We deliver physical rehabilitation in the community and promote self-management.

You will be working alongside physiotherapists and occupational therapists to provide individualised holistic assessments in the home environment for adults who have a range of physical health needs including some complex and long term conditions, formulating rehabilitation plans to assist service users in achieving their goals. There are a wide variety of conditions seen including orthopaedics, neurological, musculoskeletal and prevention and treatment of falls.

Training will be provided, along with regular supervision, in-house training, and the opportunity to attend relevant courses as highlighted in your personal development plan. You will have GCSE Maths and English, and a Foundation Degree in Health or Social Care, or be prepared to work towards this.

You will have rehabilitation experience and will need excellent communication and interpersonal skills and enthusiasm for working as part of a multi-disciplinary team
