An exciting opportunity has arisen for a Frailty Advanced Clinical Practitioner to join our Frimley Health Team. We are looking for an exceptional senior nurse or allied health professional that is experienced, innovative and committed to excellence.

The successful candidate must have: recent clinical experience at band 7 or above working with older people living with Frailty and / or multiple comorbidities. The candidate must possess a proven record of: working autonomously to assess patients and plan care, including independent medical prescribing, team working and leadership; time management and the ability to prioritise: delivery of education and training; continuing professional development; and service development and improvement.

It is essential that the post holder is resilient, and an excellent communicator both spoken and in written word. The right candidate will have strong interpersonal skills and be proactive in management of change.

The Frailty Advanced Clinical Practitioner will support the Consultant Practitioner for Older People in providing and ensuring exceptional clinical care to patients and leading and developing the Advanced Clinical Practitioner role within Older People’s Medicine. The post holder will be required to work collaboratively with other departments and services, medical colleagues and the Frimley Integrated Care System to continually improve quality, safety, outcomes and efficiencies for older adults. This will include the provision of:

Clinical practice: To deliver high quality care to patients, maintain professional expertise, and exercise a high degree of personal autonomy; spending at least 80% of time in direct contact with clients, patients or communities.

Education, training and development: To deliver training to colleagues and professionals across the system to support learning, professional development and promote excellence of practice. To act as mentor to junior colleagues within the team and facilitate visits from internal and external health care professionals seeking to extend their knowledge of frailty services

Professional leadership and : To support the Consultant Practitioner to lead and influence the strategic development of person-centred, compassionate, safe and effective care and the on-going transformation in changing contexts; while facilitating collective leadership capability and capacity.

Practice, service development, research and evaluation: To facilitate the development of evidence based professional practice for self and colleagues . To undertake audit of standards of care, patient satisfaction and service delivery to inform service development and transformation of services for older patients living with frailty
