Primary Care Home (PCH) Community Staff Nurse - Full Time (37.5 hours )

We are currently looking to offer an enthusiastic and motivated Community Staff Nurse to join our team of community nursing staff working within PCH Waveney providing care at home to patients registered with GP surgeries in Waveney. The post holder will be an autonomous practitioner that is responsible for their own caseload and take a lead in the delivery of projects to improve service delivery, patient safety and quality of care.

The post holder will be based in Southwold/Kessingland but will need to be able and willing to support the other bases at Beccles/Bungay & Halesworth.

The Primary Care Home (PCH) is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centered care to adults in their own homes, residential care or clinic settings, using a health coaching approach to support patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working. The successful candidate will be accountable for the effective management of a caseload, ensuring the delivery of high standards of evidence based nursing care. Demonstrate excellent leadership, organisational and communication skills and the ability to influence and manage change.

Nursing is challenging, varied and rewarding; the service has to be flexible to meet the needs of patients. Knowledge of local and national initiatives and evidence of lifelong learning and personal development are essential.
Applicants will be a registered nurse; ideally with community experience and a desire to develop their skills. The successful candidate will be able to demonstrate how they embody our values and commitments and a desire for personal and professional development.

The PCH is commissioned to provide a 24 /7 service. Applicants will be required to participate in working shifts covering 0700 to 2030 as part of the role including weekend and bank holiday working on a rotational basis.
