An exciting opportunity has arisen and we are looking to appoint two committed, highly motivated, enthusiastic and skilled Band 6 Specialist Physiotherapist to join our, dynamic, integrated multi- disciplinary team. This post is for HCPC registered Physiotherapist with proven clinical skills and knowledge and an understanding of the safeguarding adults agenda.

The successful applicant will be dynamic, innovative and motivated with an understanding and passion for multidisciplinary working supporting the further development of collaborative working between Oxleas NHS Foundation Trust, Royal Borough of Greenwich and Lewisham and Greenwich NHS Trust.

The Joint Emergency Team is a multi-professional team consisting of Nurses, Occupational Therapists, Physiotherapists Social Workers and a GP. The team aims to avoid unnecessary presentation to hospital by supporting community patients who are in crisis as well as facilitating discharges from the Emergency Department.

The team provide holistic health and social care assessments for people over the age 18 years, who are registered with a Greenwich GP and / or live in the Borough of Greenwich. The team is based at the Woolwich Centre and operates 7 days a week, from 08.00 to 20.30, providing a rapid response to vulnerable people, to assess and determine appropriate intervention.

The team act as a liaison between acute and community settings, by identifying the most appropriate onward referrals and initiating integrated care pathways, ensuring seamless provision of care and on- going case management. Referrals are received from Health and Social Care professionals, Medical Teams, General Practitioners and Ambulance Services.

The post holder will undertake holistic assessments of patients with the aim of facilitating safe and timely discharge from ED and other non -admission areas by identifying and referring onto most appropriate community health and social care pathways. This ensures timely and appropriate patient referral to integrated community services and seamless patient care pathways and community case management.
