Full and Part time Senior Physiotherapist postiions in our Discharge Hub.

Solent NHS Trust Adult Services Portsmouth teams manage Community Services within Portsmouth, playing a pivotal role in integrated Health and Social Care delivery. Due to an exciting expansion within our service line, we have an opportunity for a Physiotherapist to join our Discharge Hub.

We are looking for a dynamic and innovative Physiotherapist for our Discharge Hub based at St Mary’s Health Campus, Portsmouth. The Discharge Hub is part of our Portsmouth Rehabilitation and Reablement Team, assisting patients in hospital to have a speedy discharge back into their own homes, together with ongoing community support if needed. The role involves working with AHP, nursing and social care colleagues in the acute environment and the community, assessing for and providing a coordinated plan to ensure a safe discharge from hospital to community services, following the principals of ‘home first - why not home, why not today?’

The successful applicant will be committed, motivated and reliable, who can demonstrate their ability to provide high quality health, care, support and information to service users and their families. You will need excellent clinical assessment and treatment skills, including positive risk assessment, as well as excellent interpersonal, team and organisational skills. There will be a requirement to supervise and develop other staff within the team. In return for your skills, we will offer regular supervision, internal and external learning and development opportunities.

Successful candidates will be expected to cover shifts between 8am to 8pm, Monday to Sunday, 365/6 days per year and will be required to travel between bases at St Mary’s Hospital and Queen Alexandra Hospital. Applicants will be required to hold a driving license and have access to appropriate transport for travel between sites.
