Senior community nurse Crisis response and Community co-ordination centre

We are looking for a Band 6 Nurse to join our Crisis response team based in the community co-ordination centre (CCC) .

The current availability is for a permanent part time member of staff, 37.5 hours per week.

The post holder will be based from the CCC at Milford Hospital, but work across Guildford and Waverley localities.

You will be assessing patients who are at high risk of admission to hospital and require holistic face to face assessment, including use of physical assessment skills, within 2 -4 hours of referral.

You will be responsible for putting in place and monitoring a comprehensive care plan for up to 72 hours.

You will be expected to work with the CCC/CR lead clinicians, therapy team leader’s community nursing leads, social care team and the wider system to ensure a smooth handover to the most clinically appropriate service.

The post holder will have Professional responsibility for themselves and those who they supervise. They will be expected to contribute to the on-going development of clinical practice, standards, research, as well as policies, protocols, guidelines and service development.

The post holder should have previous experience working with patients who have long term complex conditions, which may also include frailty.

You will provide a timely, safe, and effective response for patients in Crisis from triage to discharge.

You will need to be able to work autonomously as well as part of a multidisciplinary team and be able to promote innovation and improvement within the community therapy team. You must be well organised and an excellent communicator and be willing to work flexibly. Good time management skills are also essential.

Applicants must have NMC registration to be considered for this post and have had experience working within the NHS in a clinical role. You will need to be able to travel independently between sites and to patient’s homes.
