Are you an Advanced Nurse Practitioner (ANP) who wants to influence how nurse-led care is provided in community hospitals?

This post offers you the opportunity to use your clinical skills and knowledge in the community hospital setting, to help shape the model of care for our patients and be part of the development of this service.

Here at Sussex Community NHS Foundation Trust (SCFT) we are a very welcoming team, delivering rehabilitation and palliative care to the patients within Uckfield, Crowborough and Lewes Intermediate Care Units. Uckfield has 14 beds, Crowborough has 18 beds and Lewes has 26 beds.

The successful candidate will need to be able to be able to work autonomously, providing advanced clinical nursing skills and prescribing competencies working alongside our nursing, therapies and medical staff. Although based at a named site, there will be a requirement at times of leave, exceptional pressures and/or circumstances when it will be necessary to support other community hospital sites. Duties will include history-taking, physical assessment and prescribing to ensure our patients receive excellent care and that our beds are used effectively.

All our units accept both ‘step up’ (community referrals) and ‘step down’ (from both local and tertiary acute Trusts). The post holder will assess, diagnose and prescribe for patients with complex health needs, managing the episode of care as appropriate, as well as liaising with acute and community colleagues as required.
