Community Support Worker (CSW) Post for Telford & Wrekin CRHT

There is a vacancy in the Telford & Wrekin Crisis Resolution and Home Treatment CRHT for a Band 3 Community Support Worker.

The Telford & Wrekin Crisis Resolution & Home Treatment Team is based at Hall Court, Telford, serving the County of Telford &Wrekin.We provide a 24 hour, 365 day service to people who are experiencing a Mental Health Crisis. This support aims to prevent hospital admission and encourage early discharge from hospital through robust assessment, care planning and care delivery.

Working as part of the team the post-holder will provide support and spend therapeutic time with an allocated group of people with acute mental health needs. This is to promote the persons recovery and to assist them to maintain their place in the community and manage transitions between services. The post holder will also assist the Shift Co-ordinator/ Lead Professional to assess, plan, implement and evaluate individualised care plans. The role will require people to be able to drive; work days, nights and weekends on a rota basis.


IT skills and car driver essential. Requests for part time hours / job share may be considered if deemed appropriate by the appointing manager.
