Operational Team Lead - 37.5 hours per week

Are you looking for a new challenge, and a chance to expand your knowledge and skills? We are currently looking for an experienced Nurse or Allied Health Professional to be the Operational Team Lead for our Horsham Primary Care Network (PCN) team based at Horsham Hospital.

You will be pivotal in delivering high quality care within the community setting by working with patients proactively to enable and promote self-management and independence throughout the continuum of care.

If you are passionate about providing high quality, holistic and safe care in the community then leading our Horsham PCN team will be an opportunity for you to use your existing skills, gain new ones and be part of a great team. The Operational Team Lead will have responsibility for the ongoing clinical and operational performance, along with the financial management of the service. They will ensure effective, efficient resource utilisation to enable quality, evidence-based patient care within a safe service. This role will be supported by the Clinical Service Manager.
