Band 3 Senior Community Rehabilitation Assistant.
North West Surrey Adult Services, CSH Surrey.

We are looking for enthusiastic and motivated individuals with experience in health and social care to join our highly regarded Rapid Response Service, based at St Peter’s Hospital, Chertsey.
We provide responsive NHS community services to patients across North West Surrey as part of an integrated service with Surrey Adult Social Care Teams.

Working as a member of the multi-disciplinary Rapid Response team you will work with patients within an agreed level of competence, who may require management and/or rehabilitation interventions in their own environment to prevent acute hospital admissions and to support complex hospital discharges. You will also support the Band 2 Community Rehabilitation Assistants and assist with team Administrators in maintaining the smooth running of the team.

Although expected to complete home visits independently, you will not be alone. You will be working under the guidance and supervision of a qualified health professional i.e. Nurse, Occupational Therapist, Physiotherapist, to carry out the intervention.

The qualities we are looking for in successful applicants are:
• Excellent communication and time management skills.
• Flexible approach to work with the ability to work across a seven day service.
• Ability to work both independently and also as part of the wider multidisciplinary team, within an agreed level of competence.
• Willingness to undertake ongoing professional development and a commitment to providing high quality, safe and effective care that meets the needs of patients.
• Car owner with appropriate license to drive in the UK for business purposes.

Please note there could be a change of base in the near future which can be discussed at interview.
