Advanced Clinical Practitioner - Rapid Response Band 8A

We are currently looking to recruit dynamic, enthusiastic, innovative, forward thinking and flexible ACPs who will join our new Rapid Response Service to assist in the prevention of avoidable admission, in line with the overall objectives of the service and NHS plan.

The post holder will be expected to work multi professionally, support other team member’s roles and contribute to the future development of the Rapid Response Service.

Wiltshire Health and Care have Posts available in 3 localities across Wiltshire this is our opportunity to provide care closer to home, avoiding admission to hospital wherever possible and appropriate. We are proud that we are doing more for our patients in their own homes.

The successful candidates will be based in one of our three localities:

North Locality – Chippenham, Calne Corsham Box, Marlborough, Malmesbury

West Locality – Trowbridge, Warminster, Devizes, Melksham

South Locality – Wilton, Amesbury, Salisbury City

As an Advanced Clinical Practitioner you will be a Registered Nurse or Paramedic. You will need to have the desire to work autonomously to assess and treat patients. You will have excellent clinical assessment and decision-making skills using the full range of advanced practice skills to ensure an optimum outcome for patients providing high quality clinical assessment and treatment intervention. As an Advanced Clinical Practitioner, the post holder will be expected to support the clinical leadership and to be a role model and mentor to other clinical staff. You will be required to be a non-medical prescriber as well as to respond to deteriorating patients. You will be working across a shift system which includes evenings, weekends and bank holidays between the hours of 8-8.

You will be joining a multi-disciplinary team who pride themselves on their excellent team working and support of each other whilst treating a diverse range of conditions. You will have excellent opportunities for learning and development with access to a wide range of courses to develop your clinical skills.

As a Senior Clinician you will be able to demonstrate good leadership skills and the ability to independently make sound clinical and leadership judgements. You will regularly supervise junior colleagues.

This role will provide you with a challenging but rewarding opportunity to utilise your advanced skills and to deliver excellent care to a wide group of patients. We are happy to consider full or part time hours.
