The High Peak and North Dales Older Adult Community Mental Health Team has a vacancy for an enthusiastic Band 6 Lead Nurse RMN to cover the Derbyshire Dales area.

You will be based at the Ritz Building in Matlock and are expected to be able to drive.

Experience with older adults with organic and functional illness is essential.

This post primarily undertakes the role of Care Co-ordinator for older adults who are experiencing severe and enduring, mental ill-health.

You will undertake initial assessments, triage referrals as duty worker and take on leadership roles.

As a Lead Nurse you will demonstrate the ability to make complex decisions to formulate detailed assessments and person-centred care.

It is essential you have good communication and IT skills, and operate as a positive integrated member of the MDT.

You are expected to demonstrate care, compassion and interest with thorough Knowledge of Care Programme Approach, Mental Capacity Act and MHA with an ability to demonstrate expertise in risk management.

We are looking for experience and commitment, together a desire to make our service the best we can through innovation & collaboration with Key stakeholders.

In the move towards 'joined Up Care' you will be supported to attend CST meetings.

The team also has a 'Care home project' supporting local care homes.

As a Lead Nurse, you will have to demonstrate RMN post-registration experience & develop post-registration study & development. Also expertise around NMC competencies.

You will be expected to provide supervision to the team and students.

The team is supported by MDT.

Personal development will be supported & encouraged through IPR and regular supervision also access to training opportunities.

DBS Check
The cost of undertaking a DBS (Disclosure and Barring Service) disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure which will be deducted from your first month’s salary. The current price of a standard check is £23 and an enhanced check is £40.
