The Learning Disability Service in Derbyshire has an exciting opportunity and a brand new role for a band 5 Registered Learning Disability Nurse to come and join our innovative LD crisis team - The Intensive Support Team.

You will be working within a service that has been rated as “Good” in 2020 by the CQC.

Our ethos is to work intensively and creatively as an MDT to make a real difference to the lives of Adults with a Learning Disability and/or Autism.

We work intensely for short periods of time with people with an LD/ASD to support them at times of crisis and prevent placement breakdown or hospital admission.

We promote person centred practice and work creatively to improve people’s quality of life. The work is mainly community based with some in-reach work into hospitals when required.

The team are working flexibly and this includes being based in the office and also home working.

Derbyshire Healthcare NHS Foundation Trust has been very supportive during the Covid-19 pandemic and we strike to work in different ways to meet the outcomes for the service user, the staff and the service as a whole.

To achieve the best results for the people we support, we carry a small caseload compared to community teams.

This enables us to wrap around service users and their families/ carers and make a real difference in a short period of time before supporting people onto mainstream or community services.

The team (IST) work 8am-8pm Monday to Friday and 9-5 Saturday-Sunday on a rota basis. This rota also includes an on-call provision.

There is also an expectation to work with those service users who have a diagnosis of ASD in the absence of LD alongside our colleagues in Adult Mental Health and support people aged 16+ who are struggling with the transition to adult services.

The post offers an opportunity for you to develop into the Nurse you aspire to be; with training, supervision, and an ethos to develop our staff and work within a culture of learning.

DBS Check
The cost of undertaking a DBS (Disclosure and Barring Service) disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure which will be deducted from your first month’s salary. The current price of a standard check is £23 and an enhanced check is £40.

We are obliged to check employment history for at least the past 3 years for successful candidates. Please ensure that the referees you provide us cover at least the past 3 years of your employment/training.

Disclosure and Barring Service Check
