Home Treatment Practitioner

We are looking for an enthusiastic individual to join our Adult Home Treatment Integrated Service across the Access Team and Inpatient Areas. This developing integrated service aims to support safe and effective timely discharge from the ward with transition support and ongoing care, in addition to preventing admission with intensive support in the community where risk allows.

 

This post is part of the Access Team and will be supervised and supported in the development of the successful candidate and the service as a whole.

All positions are subject to the receipt of satisfactory references and medical clearance. Evidence of valid professional registration and appropriate DBS clearances will also be required where relevant. All posts will also be subject to successful completion of a 12 week probationary period.

 

Please note that all new employees joining NAViGO will be able to contribute to the NHS pension.

Tier 2 Certificate of Sponsorship
